Dr. Ann Marie D'Angelo, CEO and Co-Founder of Family Care Connections, LLC is a certified psychiatric clinical nurse specialist and geriatric specialist and holds a Doctorate of Nursing Practice from Chatham University in Pittsburgh Pennsylvania.

Dr. Ann Marie has a Masters Degree in Psychiatric/Mental Health Nursing from Columbia University, a Bachelors Degree in Nursing from Adelphi University, and an Associates Degree in Nursing from Nassau Community College. She specializes in geriatric and advanced psychiatric care.

Dr. Ann Marie has a private practice in Nurse Care Management, Geriatrics and Psychotherapy offering expert care management and consultation for over thirty years. Dr. Ann Marie provides services to individuals and families and assists in short and long term care planning and is typically asked to intervene in complex, clinical situations to clarify options, identify resources and advocate for services needed in order to implement a caring and thorough plan.

 

Dr. Ann Marie uses research-based tools to identify healthcare needs. Dr. Ann Marie is New York State Certified to administer the PRI and Screen used to determine nursing home placement and frequently administers the Mini-Mental Status exam as well as other tests to evaluate care levels and needs. Dr. Ann Marie determines long and short-term plans based on these evaluations to help clients remain at home or find the most appropriate place to receive care and treatment.

Dr. Ann Marie is an adjunct professor at Adelphi University with many years of experience teaching graduate, undergraduate and continuing education courses. Dr. Ann Marie has taught Nursing at Adelphi University, NYU, Nassau Community College, and the Borough of Manhattan Community College. Dr. Ann Marie is a past board member and continuing active member of Sigma Theta Tau, the nursing honor society and has an ongoing membership in APNA, the psychiatric nursing association and a tri-level membership in the American Nursing Association. Dr. Frank G. D’Angelo, JD, PhD, is Co-Founder of Family Care Connections, a graduate of Brooklyn Law School Juris Doctor (1985), and Brooklyn College CUNY BS in Psychology (1981). He also holds an MA (2021) and PhD in Christian Counseling (2022) from the Newburg Theological Seminary. Frank is a certified spiritual director and ordained Deacon in the Roman Catholic Church (2000).

 

For over 30 years Frank has been a practicing attorney in New York State concentrating in Elderlaw, Trusts and Estates, Estate Planning and Real Estate. Frank serves as Director of Medicaid, Medicare and Eldercare Consulting Services. A licensed Real Estate Broker, Frank holds certifications as GRI, CIREC, PSA, SRS, SRES. Frank specialized in issues involving Real Estate and Housing Options for Aging and has lectured and published extensively on the many challenging aspects of Eldercare and Aging.
