Dr. Ann Marie D'Angelo CEO and Co-Founder of Family Care Connections, LLC is a certified psychiatric clinical nurse specialist and geriatric specialist. She holds a Doctorate of Nursing Practice from Chatham University in Pittsburgh Pennsylvania.
She has a Masters Degree in Psychiatric/Mental Health Nursing from Columbia University and was a Rockefeller Fellow at NYU in the Nursing Doctoral Program. She has a Bachelors Degree in Nursing from Adelphi University and an Associates Degree in Nursing from Nassau Community College.
She is an adjunct professor at Adelphi University with many years of experience teaching graduate, undergraduate and continuing education courses. She has taught Nursing at Adelphi University, NYU, Nassau Community College, and the Borough of Manhattan Community College. She is a past board member and continuing active member of Sigma Theta Tau, the nursing honor society. She has an ongoing membership in APNA, the psychiatric nursing association and a tri-level membership in the American Nursing Association.

She is New York State Certified to administer the PRI and Screen used to determine nursing home placement and frequently administers the Mini-Mental Status exam as well as other tests to evaluate care levels and needs. Dr. D'Angelo determines long and short term plans based on these evaluations to help clients remain at home or find the most
appropriate place to receive care and treatment.
